Bask Onboarding
Lets now setup your Bask account. You should have already created an account. If not, please do so now. From here, you can complete the setup of your account by just following the onboarding steps. If you do skip the onboarding, you can always complete it later by clicking on the setup guide flags in the dashboard.

Phone Verification
Please enter your personal phone number to verify your account and identity.
Tell us about your business
Please provide us with some basic information about your business. If this is a new business, no worries, just provide us with the best information you have.
For business structure, please select the one that best describes your business. If you are unsure, please select "Sole Proprietorship".
Business structures are decided usually when you form your business with the state. Common examples of business structures are:

- Corporation
- Limited Liability Company (LLC)
- Non-Profit
Business Details
Now we need to know more about your business. This information enables us to underwrite your business for payment processing, electronic prescribing and dispensing of prescription medication. Don't worry, your data is encrypted end-to-end and is not accessible to anyone without your prior authorization.
Please provide us with the following information:
Legal Business Name: This is the name of your business as it appears on your business license. If you do not have a business license, please provide us with the name of your business as it appears on your bank account.
EIN: This is your Employer Identification Number. If you do not have an EIN, please provide us with your Social Security Number.
Business Address: This is the address of your business as it appears on your business license. If you do not have a business license, please provide us with the address of your business as it appears on your bank account.
Identity Document: This is a document that proves your identity. This can be a driver's license, passport, or other government issued ID. Please make sure that the name on the document matches the name on your business license or bank account.
Product Description: This is a description of the products or services you will be selling. This is used to determine if your business is eligible for payment processing.
Personal details
Now we need to know more about you. This information (about someone authorized to sign on your organization's behalf) enables us to underwrite your business for payment processing, electronic prescribing and dispensing of prescription medication. Don't worry, your data is encrypted end-to-end and is not accessible to anyone without your prior authorization.
Please provide us with the following information:
First and Last Name: This is the name of the person who is authorized to sign on your organization's behalf (probably you if you are reading this). Please ask the right person to complete this information.
Date of birth: Please select the date that matches your government issued ID from the date picker.
Home address: This is your primary address, the location you spend most of your time. This is used to verify your identity, so it should also be the address that matches your government issued ID.
Last 4 digits of SSN: The last 4 digits of your social security number. This is used to verify your identify and is necessary for payment processing and our compliance review.
Account for payouts
We need to know how you would like to receive your payouts. This information enables us to send you your payouts (where do you want the money printer to print). This is not used for any Bask related subscription payments, and is only stored for the purpose of depositing your earnings. Don't worry, your data is encrypted end-to-end and is not accessible to anyone.
Please provide us with the following information:
Routing Number: This is the routing number of your bank account (9 digits). This is sometimes referred to as an ABA number or SWIFT number.
Account Number: This is the account number of your bank account and is used to send you your payouts (12 digits).
Customer support details
How do you want your patients to get in contact with you in case they need any kind of support (non medical)?
This information will help customers recognize your business. The support details can be used by the customer in case they need any kind of support. Also the information may be visible in payment statements, receipts and throughout your website.
Please provide us with the following information:
Statement descriptor: This is the name that will appear on the customer's payment statement. This is limited to 22 characters.
Shortened descriptor: This is the shortened name that will appear on the customer's payment statement. This is limited to 12 characters.
Customer support phone number: This is the phone number that will be used by the customer to contact you in case they need any kind of support.
Customer support email address: This is the email address that will be used by the customer to contact you in case they need any kind of help.
Customer support address: This is the address that will be used by the customer to contact you.
Doctor Group
Here we need to know if you will be using our doctor group or your own.
If you are using our doctor group, our doctors are licensed in 50 states and allows for a more seamless experience. Everything will be handled by our doctors and you will not have to worry about anything. No need to manage doctor licenses, setting up physician infrastructure, or your payments flow. When using Bask's doctor group, you will be able to focus on your business and less on the compliance complexity.
If you already have your own doctor group, you can select to use it here. Later, you can also switch to our doctor group, or use a combination of both.
For example, if you have a doctor group licensed in 5 states, you can see all patients that come to you from those 5 states, and send the remaining patients to our doctor group. This way you can use our doctor group for patients that are not covered by your doctor group, without limiting the places where you can see patients. This can be configured in the settings page later.
it adds a bit more complexity to the process and also increases the cost of the service. You will have to manage your own doctor group and also manage the doctor's licenses.
If you are using your own doctor group, We also need following information:
Pharmacy Name: This is the name of your pharmacy. This is used to identify your pharmacy.
Primary Phone: This is the primary phone number of your pharmacy.
Clinic Address: This is the address of your pharmacy. Used to identify your pharmacy.
Pharmacy Fulfillment Center
Here we need to know if you will be using our pharmacy fulfillment center or your own.
If you are using our pharmacy fulfillment center, our fulfillment center is licensed in all 50 states and allows you to focus on your business and less on the logistics. Along with this, it will update the order statuses, handle refills, use custom packaging and can even receive your drug in bulk. No additional information is required.
If you are using your own fulfillment center, the management becomes heavy and you will have to manage the logistics yourself. If you decide to go with this, please provide us with the following information:
Pharmacy Name: This is the name of your pharmacy. This is used to identify your pharmacy.
Address: Address of the pharmacy will get autofilled by the pharmacy name.
